1. Churches complete: Pre-Registration and Church Group Registration Form.

2. Individuals complete: Registration, Release Form, and Camper Agreement.

3. Individuals return to Registration page and repeat step 1 for additional campers.

4. Downloadable registration forms are available but online registration is encouraged.

WHAT TO BRING FOR AT CAMP REGISTRATION

1. Completed church group registration form (see church forms available on forms page).

2. Copy of insurance card for each camper and counselor.

3. Completed adult background check for each counselor.

IMPORTANT TO NOTE:

MEDICAL PERMISSION FORMS
In compliance with Washington state laws, an extra medical permission form, from your doctor (in addition to what is on the camper registration form) is needed for every camper who takes medication or who anticipates needing medication Without this permission from your doctor the camp nurse will not be allowed to administer any medication whatsoever. This will specifically apply to campers with (but not limited to) allergies, asthma, seizures, and etc.

If there are no special medical considerations, then the medical form in the camper registration sheet will be sufficient, along with a copy of the camper's Medical Insurance Card. However, please remember that the camp nurse still may not administer medication of any kind without special written permission from the camper's health care provider.
